This guide is for surgeons and clinical teams planning Hip Resurfacing Arthroplasty (HRA) cases in CustoMED. It covers the anatomy, the workflow and the measurements you will see.
Anatomy
Femur: the operative side, where the patient-specific guide is designed.
Pelvis: used for the acetabular cup plan and for orientation (the pelvis itself has no guide).
When the opposite hip is in the scan, it is included in the segmentation and used for the leg length comparison in the plan report, so scan coverage of both hips is important.
Workflow at a glance
Case Details: create the case and upload a CT covering both hips.
Segmentation: review the 3D models of the femur and pelvis.
Landmarks: review and adjust the anatomical reference points on the femur and pelvis. See Landmarks & Anatomical Orientation.
Surgery Planning: review and adjust the position of the femoral implant and the acetabular cup. A combined view shows both bones together.
PSI Design: review and adjust the femoral guide.
Approve: review and approve the final output, then download the files and the plan report.
Plan Only vs. Plan + PSI
HRA supports Plan Only: surgical planning without producing a guide. When you create the case, choose Plan Only instead of Plan and PSI in the Case Output Configuration field of the Case Details section. See Plan Only vs. Plan + PSI.
Measurements you will see
Preoperative measurements appear in the Landmarks step and planned measurements appear in Surgery Planning.
Femur: Native Neck Angle, Neck Angle, Neck Version, Global Version, Native Femoral Head Diameter, Head Offset, Reaming Distance, Femur Leg Length, K-wire Entry To Lateral Cortex, COR.
Pelvis: Native Acetabular Inclination, Native Acetabular Version, Native Acetabular Rim Radius, Medial Acetabular Wall Thickness, Acetabular Cup Inclination, Acetabular Cup Version, Reaming Depth, Bone Thickness - Post Reaming, Implant Sitting Percentage, COR.
Combined view: Global Offset, Leg Length Discrepancy.
Tips for HRA
Make sure both hips are in the scan so the opposite side is available for comparison.
Review femoral neck orientation carefully: it drives guide fit.
