HRA demo walkthrough: Hip Resurfacing Arthroplasty from start to finish

Follow the HRA demo case step by step, from case creation to the approved plan report.

This walkthrough takes you through the Hip Resurfacing Arthroplasty (HRA) demo case from start to finish: creating the case, reviewing segmentation, planning the femoral head and the cup, designing the femoral guide and approving the case. Each step has a short animation so you can follow along on your own screen.

The demo uses a sample scan and runs only in your browser session, so you can try anything without affecting real cases.

Note: Demo cases have no Save button. Your progress is kept in your current browser session only, and approving a demo case does not use a case from your organization's case allowance.

The steps at a glance

  1. Open the demo and create the case.

  2. Review the segmentation.

  3. Plan the femur: Landmarks, Surgery Planning and PSI Design.

  4. Add the pelvis and plan the cup: Landmarks and Surgery Planning.

  5. Approve the case and review the plan report.

1. Open the demo

  1. On the home page, find Create New Case.

  2. On the Hip Resurfacing Arthroplasty (HRA) card, click Demo in the top-left corner.

The demo opens on the case creation page with the sample scan already loaded.

Note: If you don't see a Hip Resurfacing Arthroplasty card, HRA is not enabled for your organization. Contact CustoMED support to turn it on.

2. Create the case

  1. Under Surgery Details, check the sample Surgery Date, the Approach and the Operational Side. The demo uses Medial Direct Anterior; open the list to see the other option, Posterior. The side is set by the sample scan. Click Next.

  2. Under Case Details, the Case Output Configuration is Plan and PSI.

  3. Click Create Case in the top-right corner. CustoMED downloads the scan files and opens Segmentation.

Checking the HRA Surgery Details and the Approach list, clicking Next and Create Case, then Segmentation opens

3. Review the segmentation

Segmentation shows the 3D model and the CT scan in four views: 3D, Axial, Coronal and Sagittal. The panel on the right lists the segmented bones: Pelvis and Femur, each on the left and right side. The operating side is marked (Operating Side).

  • Hover a bone in the list to show its eye button, which hides or shows that bone.

  • Scroll over a CT view to move through the slices. The bone outlines follow each slice.

4. Find your way around the editor

After segmentation, the editor opens on the femur. A few controls stay the same in every step:

  • The icons on the left switch between Femur view, Pelvis view and Combined view. Combined view is available in the Landmarks step; step 9 shows it.

  • Click the step name in the header (for example Landmarks) to open the step menu. It lists every step, and completed steps have a check mark.

  • The back arrow next to the main button returns to the previous step.

  • Drag the Internal / Surrounding slider at the top of the 3D view to show the CT scan around the model.

  • Use Sagittal, Coronal and Axial under the 3D view to jump to a standard viewing angle.

5. Femur: Landmarks

CustoMED places the femur landmarks for you. Review them before you plan.

Review the measurements and landmark groups

  1. Check Pre-Op Measurements for both sides: Native Neck Angle Varus, Global Version, Native Femoral Head Diameter (mm), Head Offset (mm) and Femur Leg Length (mm).

  2. Expand Femur on the operating side to see the landmark groups: Anatomical Axis, Trans Epicondylar Axis (TEA) and Femoral Neck Axis, plus Fovea and Lesser Trochanter Tip. Hover a row to preview it.

6. Femur: Surgery Planning

Surgery Planning places the resurfacing head on the femur, with a K-wire through the femoral neck.

Adjust the measurements

  1. Check Post-Op Measurements: Neck Angle Varus, Neck Version, Global Version, Head Offset (mm) and Reaming Distance (mm), followed by Femur Leg Length (mm), K-wire Entry To Lateral Cortex (mm) and COR (mm).

  2. Use the minus and plus buttons next to the first five to change them. The head moves to match, and the other values update.

  3. Switch between Pre-Op and Post-Op at the top of the panel to compare the native femur with the plan.

Check the resurfacing head

  1. Expand the resurfacing head card to change the implant: Manufacturer, Sets and Size, then click Select Template. Open Size to see the available sizes.

  2. With the card open, use the dials along the edges of the 3D view (Anteversion, Rotation and Inclination) to turn the head one step at a time.

Opening the resurfacing head card and its Size list, then turning the head with the Inclination dial

Preview the head preparation tools

The resurfacing head card also lists the tools used to prepare the femoral head: Kwire, Reamer Plane, Cylinder Cutter and Barrel. Only the K-wire is shown at first. Hover a row and click its eye button to show or hide that tool on the model.
