This walkthrough takes you through the Total Hip Replacement (THR) demo case from start to finish: creating the case, reviewing segmentation, planning the femur and the pelvis, designing the femoral guide and approving the case. Each step has a short animation so you can follow along on your own screen.
The demo uses a sample scan and runs only in your browser session, so you can try anything without affecting real cases.
Note: Demo cases have no Save button. Your progress is kept in your current browser session only, and approving a demo case does not use a case from your organization's case allowance.
The steps at a glance
Open the demo and create the case.
Review the segmentation.
Plan the femur: Landmarks, Surgery Planning and PSI Design.
Add the pelvis and plan the cup: Landmarks and Surgery Planning.
Approve the case and review the plan report.
1. Open the demo
On the home page, find Create New Case.
On the Total Hip Replacement (THR) card, click Demo in the top-left corner.
The demo opens on the case creation page with the sample scan already loaded.
Note: If you don't see a Total Hip Replacement card, THR is not enabled for your organization. Contact CustoMED support to turn it on.
2. Create the case
Under Surgery Details, check the sample Surgery Date, the Approach (Posterior) and the Operational Side. The side is set by the sample scan. Click Next.
Under Case Details, the Case Output Configuration is Plan and PSI.
Click Create Case in the top-right corner. CustoMED downloads the scan files and opens Segmentation.
3. Review the segmentation
Segmentation shows the 3D model and the CT scan in four views: 3D, Axial, Coronal and Sagittal. The panel on the right lists the segmented bones: Pelvis and Femur, each on the left and right side. The operating side is marked (Operating Side).
Hover a bone in the list to show its eye button, which hides or shows that bone.
Scroll over a CT view to move through the slices. The bone outlines follow each slice.
Click the expand button in the corner of the 3D view to make it full size, and drag to rotate the hips.
When the bones look right, click Confirm.
4. Find your way around the editor
After segmentation, the editor opens on the femur. A few controls stay the same in every step:
The icons on the left switch between Femur view, Pelvis view and Combined view.
Click the step name in the header (for example Landmarks) to open the step menu. It lists every step, and completed steps have a check mark.
The back arrow next to the main button returns to the previous step.
Drag the Internal / Surrounding slider at the top of the 3D view to show the CT scan around the model.
Use Sagittal, Coronal and Axial under the 3D view to jump to a standard viewing angle.
Combined view shows the pelvis and both femurs together. Its Pre-Op Measurements compare the two hips: Global Offset (mm) for each side, split into its Femur and Pelvis parts, and Leg Length Discrepancy (mm). The main button is unavailable in Combined view, so switch back to Femur view or Pelvis view to continue.
5. Femur: Landmarks
CustoMED places the femur landmarks for you. Review them before you plan.
Review the measurements and landmark groups
Check Pre-Op Measurements for both sides: Native Neck Angle Varus, Global Version, Native Femoral Head Diameter (mm), Head Offset (mm) and Femur Leg Length (mm).
Expand Femur on the operating side to see the landmark groups: Anatomical Axis, Trans Epicondylar Axis (TEA) and Femoral Neck Axis, plus Lesser Trochanter Tip and Fovea. Hover a row to preview it.
Check the femoral neck points and confirm
Expand Femoral Neck Axis and click Femoral Head Medial Point or Neck Axis Lateral Point to select it in the 3D view.
Use Axial, Sagittal and Coronal to check the points from different angles.
Click Confirm Landmarks. CustoMED creates the surgery plan and opens Surgery Planning.
6. Femur: Surgery Planning
Adjust the measurements
Check Post-Op Measurements: Neck Angle Varus, Global Version and Head Offset (mm), followed by Femur Leg Length (mm), Resection level to LT (mm) and COR (mm).
Use the minus and plus buttons next to the first three to change them. The stem moves to match, and the other values update.
Switch between Pre-Op and Post-Op at the top of the panel to compare the native femur with the plan.
Check the stem and head
Expand the stem card to change the implant: Manufacturer, Sets and Size, then click Select Template. Open Size to see the available sizes.
With the stem card open, use the dials around the model (Anteversion, Rotation and Inclination) to turn the stem one step at a time.
The card also lists the Neck Plane and the femoral head. Expand the femoral head to choose its Size, then click Select Template Part.
7. Femur: PSI Design
Look at the guide
Click Generate PSI. CustoMED designs the femoral guide and opens PSI Design.
Rotate the model to look at the guide on the femoral neck from every side.
Expand Psi to see the guide parts: the Psi itself, two K-wires and the Cutting Blade. Hover a part to preview it.
Check the cut and confirm
Zoom in on the femoral neck and rotate the model. The Cutting Blade shows where the neck will be cut.
Click a part, such as the Cutting Blade, to select it in the 3D view.
Switch to Post-Op to see the femur after the neck cut, then back to Pre-Op.
Click Confirm PSI. CustoMED prepares the export files and opens Approve.
8. Add the pelvis
Approve shows the output pack. At this point it holds the femur plan and its PSI, and the pelvis is marked Not planned. The main button reads Sign off while a bone is not planned, and Approve Case once every bone is in the pack.
Use the arrows on the preview to switch between the femur plan and its PSI preview, and expand Files to see what the pack will contain.
Click Add Pelvis. The editor opens on the pelvis Landmarks step.
9. Pelvis: Landmarks
Review the measurements and landmarks
Check Pre-Op Measurements for both sides: Native Acetabular Inclination, Native Acetabular Version, Native Acetabular Rim Radius (mm) and Medial Acetabular Wall Thickness (mm).
Expand Pelvis on the operating side to see the landmarks, including the Mid-sagittal Plane, the Acetabular Plane, Pubic Symphysis, Acetabular Fovea and the points around the acetabular rim.
Click a landmark to select it in the 3D view.
Check the acetabulum and confirm
Zoom in on the acetabulum and rotate the pelvis to see the rim points and the Acetabular Plane.
Hover a landmark in the list to highlight it in the 3D view.
Click Confirm Landmarks. CustoMED creates the cup plan and opens Surgery Planning.
10. Pelvis: Surgery Planning
Adjust the cup position
Check Post-Op Measurements: Acetabular Cup Inclination, Acetabular Cup Version and Reaming Depth (mm), followed by Bone Thickness - Post Reaming (mm), Implant Sitting Percentage and COR (mm).
Use the minus and plus buttons to change the first three. The other values update as the cup moves. The reamed bone is shown in red.
Check the cup
Expand the cup card to change the implant: Manufacturer, Sets, Type and Size, then click Select Template. Open Size to see the available sizes.
With the card open, use the dials around the model (Anteversion, Rotation and Inclination) to turn the cup one step at a time.
Rotate the model to check the cup from the side.
The pelvis has no guide in THR, so its last step is Surgery Planning. Click Confirm Surgery Plan to return to Approve.
11. Approve the case
Check that Output Pack 1 lists both bones: Pelvis with Plan only, and Femur with its PSI Output.
Step through the previews and open Post-Op Measurements under the preview to see the planned values.
Expand Files to see the Delivery Note, the Case Files and the Plan Report (SFU).
Click Approve Case.
Read the Responsibility Acknowledgment and click Approve & Acknowledge.
CustoMED then creates the plan report. A Generating Plan Report... bar shows the progress. Keep the tab open until it finishes.
12. After approval
The header shows Approved, and the main button changes to View Only (Approved Case).
The plan report preview appears next to the pack. Use its arrows to turn pages, or click Preview to open the full Plan Report (SFU).
Under Files, preview the Plan Report (SFU) and the Delivery Note, or download the Case Files.
That's the whole THR workflow. When you're ready, create a real case from the same Total Hip Replacement card.
