CRP demo walkthrough: Cranioplasty from start to finish

Follow the Cranioplasty demo case step by step, from case creation to the approved plan report.

This walkthrough takes you through the Cranioplasty (CRP) demo case from start to finish: creating the case, reviewing segmentation, shaping the cranial patch, designing the implant and approving the case. Each step has a short animation so you can follow along on your own screen.

The demo uses a sample scan and runs only in your browser session, so you can try anything without affecting real cases.

Note: Demo cases have no Save button. Your progress is kept in your current browser session only, and approving a demo case does not use a case from your organization's case allowance.

The steps at a glance

  1. Open the demo and create the case.

  2. Review the segmentation.

  3. Shape the cranial patch in Surgery Planning.

  4. Review the implant in PSI Design.

  5. Approve the case and review the plan report.

Cranioplasty has no Landmarks step. You plan directly on the patch that covers the skull defect.

1. Open the demo

  1. On the home page, find Create New Case.

  2. If you don't see the Cranioplasty card, click the arrow on the right of the cards to scroll.

  3. On the Cranioplasty card, click Demo in the top-left corner.

The demo opens on the case creation page with the sample scan already loaded. The Segmenting... bar at the top shows simulated segmentation progress while you fill in the case.

Scrolling the Create New Case cards to Cranioplasty, clicking Demo, then the case creation page with the Segmenting bar

Note: If you don't see a Cranioplasty card, Cranioplasty is not enabled for your organization. Contact CustoMED support to turn it on.

2. Create the case

  1. Under Surgery Details, check the sample Surgery Date and the PSI Type. The demo uses PEEK Implant. Click Next.

  2. Under Case Details, the only field is Patient Hint.

  3. Click Create Case in the top-right corner. CustoMED downloads the scan files and opens Segmentation.

Checking the Cranioplasty Surgery Details, clicking Next and Create Case, then Segmentation opens

3. Review the segmentation

Segmentation shows the 3D model and the CT scan in four views: 3D, Axial, Coronal and Sagittal. The panel on the right lists one segmented bone, the Skull.

  • Hover Skull in the list to show its eye button, which hides or shows the skull.

  • Scroll over a CT view to move through the slices. The bone outline follows each slice.

Hovering the Skull card, then scrolling through the Axial CT slices
  • Click the expand button in the corner of the 3D view to make it full size, and drag to rotate the skull until you see the defect.

  • When the skull looks right, click Confirm. The editor opens on Surgery Planning.

Rotating the full-size 3D skull to the defect, then clicking Confirm to open Surgery Planning

4. Find your way around the editor

A few controls stay the same in every editor step:

  • Click the step name in the header (for example Surgery Planning) to open the step menu. It lists every step, and completed steps have a check mark.

  • The back arrow next to the main button returns to the previous step.

  • Drag the Internal / Surrounding slider at the top of the 3D view to show the CT scan around the model.

  • Use Sagittal, Coronal and Axial under the 3D view to jump to a standard viewing angle.

5. Surgery Planning

Surgery Planning opens with a front view of the skull. The panel lists two cards: Skull and Cranial patch. The cranial patch covers the defect and is the shape the implant is built from.

Find the defect

Drag on the model to rotate the skull until you see the patch over the defect.

Adjust the patch outline

  1. Expand Cranial patch. The patch turns blue, and points appear along its edge.

  2. Under Boundary Points, use the Amount slider to choose how many points are shown along the edge.

  3. Zoom in and drag a point to move the edge of the patch. The patch follows the new outline.

Expanding Cranial patch, lowering the Amount of boundary points, then dragging a point to move the patch edge

Shape the patch surface

  1. Click Shape to open the brushes. Choose a Modification Type: Curve bends the patch under the brush, and Smooth evens out its surface.

  2. Set the brush size with Modification Diameter (mm), or hold Ctrl (⌘ on Mac) and scroll over the patch.

  3. Press and drag over the patch. Each stroke changes the patch while you drag.

  4. To take back a stroke, click the undo button at the top of the panel. Each stroke is one undo step.

Opening Shape, drawing a Curve stroke on the patch, resizing the brush, then drawing a Smooth stroke

Note: While Shape is open, pressing on the skull or the patch draws a stroke instead of turning the view. To rotate, drag on the empty background, or close Shape first.

Lay the patch onto the bone with Stitch

Stitch lays the patch onto the bone you see behind it, or pulls it out where it has sunk into the bone. It works where the bone is within a few millimeters of the patch. Stitch has its own settings:

  • Brush Size (mm): the area of the patch each stroke picks up.

  • Sigma (mm): how far the movement blends into the surrounding patch. Keep it close to the brush size for a local touch-up.

  • Iterations: how many times each touch of the brush is repeated. More repeats close the gap more fully.

Choosing Stitch and its Brush Size, Sigma and Iterations settings, then closing the Cranial patch card

6. PSI Design

Look at the implant

  1. Click Generate PSI. A progress bar shows while CustoMED designs the implant. This can take a few minutes.

  2. PSI Design opens. Rotate the skull to look at the implant in the defect.

  3. The card at the top of the panel shows the PSI type, PEEK Implant (Cranioplasty Polymeric Implant). Expand it to choose another PSI type available to your organization. Choosing a different type changes the main button to Update PSI.

Check the implant settings

  1. Expand Implant. The implant is highlighted in blue.

  2. Open Hole Type to see the options: CutThrough, Marker and None. The demo uses Marker, which shows as small marks across the implant surface.

  3. Clearance sets the gap between the implant and the skull, in millimeters.

  4. If you change a setting, the main button changes to Update PSI. Click it to design the implant again with the new setting.

Clicking Generate PSI, rotating to the implant in PSI Design, then expanding Implant and opening the Hole Type list

Check the views and confirm

  1. Collapse Implant and click Axial to look down at the implant from above. The hole marks are easy to see from here.

  2. Click Confirm PSI. CustoMED prepares the export files and opens Approve.

Checking the implant in the Axial and Coronal views, then clicking Confirm PSI and opening Approve

7. Approve the case

  1. Check that Output Pack 1 lists Skull with its plan and PSI Output.

  2. Drag the preview to rotate it, and use the arrows to switch between the plan and the plan with the implant.

  3. Expand Files to see the Delivery Note, the Case Files and the Plan Report (SFU).

  4. Click Approve Case.

  5. Read the Responsibility Acknowledgment and click Approve & Acknowledge.

CustoMED then creates the plan report. A Generating Plan Report... bar shows the progress. Keep the tab open until it finishes.

Rotating the plan preview, switching to the implant preview, expanding Files, then clicking Approve Case and Approve and Acknowledge

8. After approval

  • The header shows Approved, and the main button changes to View Only (Approved Case).

  • The plan report preview appears next to the pack. Use its arrows to turn pages, or click Preview to open the full Plan Report (SFU).

  • Under Files, preview the Plan Report (SFU) and the Delivery Note, or download the Case Files. Hover the info icon next to Plan Report (SFU) to see who created it and when.

That's the whole Cranioplasty workflow. When you're ready, create a real case from the same Cranioplasty card.
